INTAKE REQUIREMENTS ( Read All)
- To ensure legal compliance, efficiency, and accurate service execution, all clients must meet the following intake requirements before an appointment can be confirmed.
- 1. Identity Verification Requirements
- All signers must present:
- • Valid, unexpired, government-issued photo identification
• ID must be physical (not expired, not digital-only unless permitted for RON credential analysis)
• ID must include photograph, signature, and identifying information - Acceptable identification includes:
- • State driver’s license
• State identification card
• U.S. passport or foreign passport
• U.S. military identification card
• Permanent resident card (Green Card)
• Other government-issued ID permitted under Illinois law - Identification must be presented at the appointment. Copies may be requested in advance for preparation purposes.
- 2. Document Readiness Requirements
- Clients must ensure:
- • Documents are complete before the appointment
• Documents are unsigned unless instructed otherwise
• All pages requiring notarization are present
• No missing pages or incomplete sections
• Correct notarial certificate is included (if known) - Fayrely™ Services may review documents for completeness and readiness but does not review content for legality or accuracy.
- Incomplete or altered documents may result in refusal of service.
- 3. Signer Requirements
- All signers must:
- • Be physically present for in-person notarization
• Appear voluntarily and without coercion
• Be mentally aware and capable of understanding the document
• Be able to communicate directly with the notary
• Be prepared with required witnesses if applicable (or request witness procurement) - Signers who appear impaired, coerced, or unable to understand the transaction cannot be notarized.
- 4. Remote Online Notarization (RON) Requirements
- RON clients must have:
- • Computer, tablet, or smartphone with camera and microphone
• Stable internet connection
• Ability to complete identity verification and credential analysis
• Ability to complete knowledge-based authentication (if required)
• Ability to upload documents in advance - The notary must be physically located in Illinois during the RON session.
- RON sessions may be terminated if identity verification or technology requirements are not met.
- 5. Mobile and Field Service Requirements
- Clients requesting mobile or field services must provide:
- • Full service address
• Building access instructions
• Parking and entry details
• Facility requirements (hospitals, assisted living, correctional facilities, etc.) - Travel fees and facility coordination requirements will be disclosed before confirmation.
- 6. Apostille and International Document Intake Requirements
- Clients requesting apostille coordination must provide:
- • Destination country
• Document type
• Original documents or certified copies (when required)
• Any applicable authorization letters
• Deadline or urgency requirements - Processing timelines are controlled by government and foreign authorities and cannot be guaranteed.
- 7. Vital Record Retrieval Intake Requirements
- Clients must provide:
- • Signed authorization letter (notarized when required)
• Copy of requester’s government ID
• Any required proof of relationship (if applicable)
• Document details (name, date, county/state) - Fayrely Services™ acts as an authorized agent, not as the issuing authority.
- 8. Scheduling and Payment Requirements
- To confirm an appointment, clients must:
- • Submit all required intake information
• Submit required deposit (if applicable)
• Upload required documents when requested
• Accept booking and compliance policies - Appointments are not confirmed until intake and deposit requirements are completed.
- 9. Right to Refuse Service
- Service will be refused if intake requirements are not met, including but not limited to:
- • Invalid or missing identification
• Incomplete or altered documents
• Failed identity verification
• Suspected coercion or fraud
• Technology failure (RON)
• Noncompliance with Illinois law

As part of our commitment to transparency, we operate under strict Illinois guidelines for notarial acts. We provide mobile services to homes, offices, and facilities, as well as Remote Online Notarization for convenience across any distance.

APPOINTMENT CONFIRMATION REQUIREMENTS
To ensure legal compliance and efficiency, all clients must confirm the following before an appointment is finalized:
- Submission of accurate intake information and document verification.
- Payment of a non-refundable deposit to secure your preferred scheduling slot.
- All payments are required in full at the time of service. Refunds are not issued once a notarial act is completed.
SIGNER REQUIREMENTS
The integrity of the notarial act depends on the readiness of the signer:
- Signers must be physically present and appear voluntarily.
- Presentation of valid, unexpired government-issued photo identification is mandatory.
- For Remote Online Notarization (RON), signers must possess a stable internet connection and functional audio-video hardware.
- We reserve the right to refuse service if these requirements are not met; applicable administrative fees will still apply.
REFUSAL OF SERVICE
Safety and legal compliance are non-negotiable. Service will be refused if:
- Identification is invalid, expired, or missing.
- The document is incomplete, contains blank spaces, or shows signs of alteration.
- The signer appears impaired, confused, or under duress.
- Refusal of service for legal non-compliance is mandatory; travel and administrative fees are non-refundable.
DEPOSITS POLICY
Deposits reserve our specialized time and cover preliminary compliance reviews:
- Standard Appointments: Deposit is based on service and is non-refundable deposit unless stated by notary.
- Hague/Non-Hague Apostille: $50 administrative deposit.
- Deposits are applied directly to your final invoice upon service completion.
- Credit and debit cards will be charged for services rendered and any other applicable fees ( i.e late fees, cancelation fees, no-show)
CANCELLATION POLICY
We value our time and yours. Cancellations affect our ability to serve other clients:
- Notice > 24 Hours: Deposit retained as service credit for 30 days.
- Notice 12–24 Hours: Full forfeiture of deposit.
- Notice < 12 Hours: Full service fee may be charged.
- Credit and debit cards will be charged for services rendered and any other applicable fees ( i.e late fees, cancelation fees, no-show)
NO-SHOW POLICY
A 'No-Show' is defined as a failure to appear, lack of ID, or being unprepared for the session start time.
No-Show Fees:
- In-Person/Electronic: $25
- Mobile Travel: $25
- RON: $25
- Credit and debit cards will be charged for services rendered and any other applicable fees ( i.e late fees, cancelation fees, no-show)
GRACE PERIOD & WAIT FEES
- A 15-minute grace period is provided for all scheduled appointments.
- Wait fees of $15 per 15-minute increments apply after the grace period ($60 maximum).
- Appointments are subject to cancellation after 30 minutes of delay.
- Credit and debit cards will be charged for services rendered and any other applicable fees ( i.e late fees, cancelation fees, no-show)
ADMINISTRATIVE & TIME-BASED FEES (NON-NOTARIAL)
Services beyond the notarial act are billed separately:
- Printing, scanning, or secure shredding: $1.00 per page.
- Courier shipment, drop-off coordination or both: $50-$100 flat fee.
- Extended consultation or document review (>15 mins): $40 per hour.
TRAVEL & MOBILE POLICIES
Mileage-Based Travel Fees:
- 0–10 miles: $45
- 11–20 miles: $55
- 21–30 miles: $75
- 30+ miles: Customized Quote
Additional Surcharges:
- After-hours (5PM–7PM) or Emergency: +$50
- Holidays and High-Security Facilities: +$50
All travel fees are final and disclosed prior to departure.
REMOTE ONLINE NOTARIZATION (RON) COMPLIANCE
- Full compliance with 5 ILCS 312/ et seq. and Illinois Administrative Rules.
- Sessions are recorded for security and legal record-keeping.
RON Administrative Fees:
- Platform Access & Technology Fee: $25
- Notarial act fee: $25 per act
APOSTILLE & INTERNATIONAL SERVICES
- Coordination for Hague and Non-Hague documents.
- Hague Processing: $200+ | Non-Hague: $250+.
- Processing timelines are subject to government agency schedules and cannot be guaranteed.
VITAL RECORD RETRIEVAL
Fayrely Services acts as an authorized agent for record retrieval:
- Requirements: Valid ID and signed authorization.
- Agent Fee: $75 flat fee per document request.
- Government and retrieval costs are billed at cost.
FIELD INSPECTION & NON-NOTARIAL SERVICES
- Objective property and business inspections.
- Verified time-stamped photo documentation.
- Full inspection reporting and verification.
- Pricing is disclosed per scope of work.
ACKNOWLEDGMENT
By proceeding with a booking, you acknowledge that you have read, understood, and agreed to these policies in their entirety.
- I accept the cancellation and no-show fee structures.
- I understand the requirement for valid identification.
- I acknowledge that no legal advice is being provided.

PAYMENT TERMS
- Full payment for all mobile travel, administrative fees, and non-notarial services is required before or at the time of session.
- We accept secure electronic payments via Zelle and Venmo, all major credit/debit cards, and cash (exact change required). Credit and debit cards will be charged for services rendered and any other applicable fees ( i.e late fees, cancelation fees, no-show)
- Invoicing and professional billing are available for corporate accounts and legal firms upon contract verification.
- All requested administrative fees are non-refundable once the service window has commenced or the coordination process has been completed.
PROFESSIONAL STANDARDS
- We operate as a fully bonded ($30,000) and insured ($25,000 E&O) firm within the State of Illinois.
- We strictly adhere to the Illinois Notary Public Act and best practices established by the National Notary Association (NNA).
- Our team maintains continuous education on state compliance, legislative updates, and data security protocols.
- We maintain neutrality, integrity, and absolute confidentiality regarding all documents and client information handled.
FINAL ACKNOWLEDGMENT
By submitting the intake form or paying a deposit, you confirm that you have read, clearly understood, and agree to the booking, cancellation, and payment policies of Fayrely™ Services. You further acknowledge the legal disclosure that we are not licensed attorneys and do not provide legal advice.
